Achilles treatment in Surrey/London

Just over a decade ago I was training hard aiming to qualify for the Triathlon age group category (amateur racing basically). I got Achilles tendonitis/tendinosis and have pretty much been in pain since whenever I try to increase physical activity.
Have tried various local physios and stretching/strengthening programs over the years. Bit nothing has worked. I went to the GP recently... The general experience has been enough to convince me to splash out and go private.
Can anyone recommend somewhere in Surrey or London? I want the whole shebang; physio advice, scans, shockwave therapy, etc. And I want to avoid the witch doctors.

When I was a teenager I had a lot of problems with one of my Achilles.
Nothing showed up on any x-ray and scan, fortunately my dad had private medical insurance for the family and eventually the GP grudgingly gave a referral.
The surgeon was brilliant, he said, "there's obviously a problem, let's go inside and have a look."
He found and removed a bone spur rubbing on the tendon. Once the wound had healed it was perfect.
