Bladder Neck Incision (plus a little TURP on the sides)
Discussion
Now waiting for lunch after yesterday s procedure. Operation was at 11h, woke up in the recovery room at 13h30, brought up to the ward at 14h. This a public hospital, just two beds in the room, and I got lucky with the allocation

While releasing/enlarging the bladder neck muscle with small incisions the surgeon took away some of the surrounding prostate tissue (~5% of total) just to make sure.
Why all this? Last October I noticed increasing difficulty in going for wee; slow getting started, then weak flow and feeling like the bladder was still half full. GP said it was benign prostate hyperplasia (59 years old, very low PSA, normal weight, reasonably active) and prescribed a plant-based pill.
Six weeks later showed no improvement, so he referred me to a urologist whom I saw in early December. He did an ultrasound, digital examination, volume/speed tests, and prescribed alpha blockers to relax the muscles around the bladder/prostate. I then flew off to Japan for a three week holiday, where things got better (less urgency/hesitation) but still not fully emptying.
Saw the same urologist in early January, who booked me in for a cystoscopy a week later. The results of that pretty uncomfortable experience (tube with camera inserted into the bladder with local anaesthetic up the todger), showed a fairly severe bladder neck muscle constriction but an apparently normal-sized prostate.
He wrote me a referral letter to the public hospital for surgery. The choice was a 5 week wait going private with him (not covered by my basic Swiss obligatory insurance) for £16k, or 10 weeks and free in the cantonal hospital. So here I am!
I m still catheterised, but now off the main hanging rinsing bags and have a smaller bag strapped to my leg, and drinking as much water as I keep down. Catheter removal scheduled for tomorrow morning and back home by lunchtime.
Then ten days of rest and recuperation before being able to drive and going back to work. No heavy lifting for three weeks. And no (motor)cycling for five weeks
But at least by then all should be healed up and pissing like racehorse again
PS.
TURP: Transurethral Resection of the Prostate.

Yes, I had a long while to reflect, but leaving it as is for too long was not really an option, if I wanted to regain proper bladder control afterwards.
The bladder was already showing signs of stress (une vessie de lutte, they call it here), not to mention the risk of kidney issues if the constriction got worse. No meds were making any real difference at all, and the whole situation had become very tiring and frustrating.
ED is rare for BNI procedures, and in my case they took very little of the prostate itself. Dry orgasm is a 80% risk according to my surgeon. However I am nearly 60, don’t want any more kids, and if it happens, so be it. I just told Mrs B to make the most of my ‘capabilities’ in the period before the op
I hope you get good advice and find the best solution for you. I am now at home and taking it very easy for the next 7-10 days.
It’s not a minimal procedure, but the most difficult parts are more uncomfortable rather than outright painful. Just look forward to a positive outcome, retain a sense of humour, leaving most of your dignity at the door

Came off painkillers after ten days, bleeding became more and more sporadic and now twenty days later has now stopped.
Still no motorbikes for another two weeks, but feeling almost back to normal. Did a full week in the office last week. And indeed p*ssing like a racehorse again!
