I'm using an old Renault 19 as a runabout (1.4 H reg) and the gearbox has started to whine, should I expect a Vesuvial type of failure imminently

It will break eventually, but not yet. Mechanical sympathy is the answer here. smile

Mechanical poker time, gearbox could go on for ages or say "no" anytime.
You have checked oil level?
Perhaps change oil anyway and use a semi synthetic.

Worse in 1st and 2nd?

Probably slop in the laygear bearing at the 1st gear end. It causes the cogs not to mesh properly and the resulting noise. Answer, don't give it mega beans in 1st and second. When it gets to a certain point, it will strip some teeth. Is it only under power on? They can go on and on and on like it. 3rd will gradually get worse but you can give it full beans in 4th and 5th.

There isn't much you can do to shut it up without compromising lubrication.
