Cup holders.

Ones that actually work and are in a useful place. I'm tired of stopping for a cold drink then wanting to carry on my journey with cold vegetables.

Headlamps which work. One out of our three cars have lights which actually light the road. (none are xenon). I think new cars have that area pretty much under control now.

Bluetooth.... we were late for a meeting in Norway. The self designated hire car driver decided that working out the blue tooth was more important than the meeting. I wanted to take off my pointy leather loafer and smash the interface into a million pieces.

Just as bad as a mobile phone, its the distraction of the conversation, not the phone itself which causes accidents.
