Driving at a steady 70 in the inside lane, a 4x4 driven by an elderly woman signalled and pulled out from the hard shoulder at zero mph.
I anchored on, ABS kicked in and just before I hit the rear end at a still considerable speed the car on my outside went steeming past and I was able to pull out into the middle lane.
I was so much in shock I had to pull over and stop for a few minutes - needless to say I rejoined the motorway having built up my speed on the hard shoulder.
That possibly the closest I have come to a serious accident.

It was about 9pm, pissing down and the middle of winter. It had no lights on and didn't indicate. I ended up on the hard shoulder as there was someone overtaking me in the middle lane.

Not that it'd do any good mind, there was one of these in traffic cops. Despite being pulled and then specifically told to gain speed before moving over, the car pulled straight into the path of an incoming truck. Some people really do have no common sense.

Perhaps easier said than done but the hard shoulder would have been your escape route. With abs brake hard and look for a way out.
she was still half on/off the hard shoulder unfortunately. but fortunately it being only half in the inside lane it enabled me to move to the middle. I was that close, had she been fully in the lane I think I would have either still clipped her or had to steer so violently to risk myself flipping skidding etc.You have my sympathies OP. I had this happen to me at the start of the year, similar circumstances and by far the scariest thing I've had happen to me. I was in the inside lane, someone in lane 2 close behind, dozy bint joining the motorway immediately from the hard shoulder having just been stopped by traffic police. Luckily the guy in lane two clocked what was happening and slowed/flashed to let me move over just in time; I was so close to her when she pulled this move that I just missed her - didn't even get fully into lane 2.
Having seen the traffic car before the incident I obviously pulled over after what happened; so did the guy who let me out. I was shaking like nothing before. One officer came to check if I was alright/if there was damage to the car and commended the actions of the guy behind me (as did I). At the same time I could hear the other officer giving it laudy to the woman.
No idea what happened to her after that; I was back on my way and it was a very cautious drive home.

Something similar happened to me. Doing 60 in the left lane, light rain and a wet road. From a layby a youngish woman pulls out with no indication quite close in front of me and doesn't attempt to get up to any reasonable speed. My mk1 mx5 (which is in need of a geometry setup) was squirming all over the place as I stood on the brakes.
I pulled up alongside her after and she just looked at me with a wtf expression, probably didn't even realise she had put me, herself and her kid in the backseat in such a dangerous situation. Scary people so clueless are allowed to drive on the road.

I had something similar when coming home on the A9, pitch black and raining I was doing 75 just about to be overtaken by a couple of cars when I realised that someone was flashing a small torch at us from what I thought was the hard shoulder. Turns out that an HGV had completely broken down without any lights halfway on to the HS, the flash light was the driver standing behind it trying his best to warn us.
Safe to say it was lucky that my car and the one that was overtaking me were small as we passed the HGV side by side.
I couldn't believe that it was possible for a modern HGV to be completely without lights when broken down as the situation was incredibly dangerous.

Saw a Vectra do this to a HGV in my mirrors on the M6 near Stoke a few years back.
Unfortunately 44 tons is not as easy to stop or take evasive action with.
By the time I'd pulled over, retrieved my first aid kit and started running back to the mess another guy who'd stopped told me there was no point.
I'm still twitchy about anything on the hard shoulder to this day.

Same here these days, whenever possible i'll pull into the middle lane on a motorway to safely pass whatever is on the hard shoulder.Unfortunately 44 tons is not as easy to stop or take evasive action with.

Old bidy exits the roundabout onto Lane 2 of oncoming DC, we are approaching and she then turns right, across our path, into a side road.
Still not sure if it was intentional or not.
Trouble is people sit in their own little world completely oblivious to what is going on.
Like today, 3 lane road, 2 lanes going in my direction. Person in a black Honda is sitting in Lane 2 bimbling along, I am on the left side of Lane 1 and also bimbling with a grey car, possibly a Daihatsu, in front. Ambulance on Blues and twos is coming the other way, closing on a car, and pulls into my sides Lane 2 - black Honda barely notices to get out the way in time and almost has a 3 into 2 don't go moment - maybe a foot spare gap between oncoming car, Ambulance and Honda.
The road in question at black Hondas position had visibility for at least half a mile had they been bothering to look, and I saw the ambulance through its windscreen...
