An insurance related question following an accident

I have a question for the more insurance minded about renewing insurance following an accident.
Here is the scenario: We have 2 cars:
1. MG ZR - V5 in the missus name, and insurance policy in mine, 5 year protected no claims.
2. Renault Laguna (now a Micra following the accident I am about to write about) where I have V5 and policy is in SWMBO's name, 0 no claims.
Late November I was driving the Laguna and got T-boned at a traffic light controlled crossroads. I was turning right and misjudged the gap (I'm certain he was travelling faster than the posted 20, but s

So I called SWMBO's insurers and reported it, didn't make a claim myself and then we scrapped the car and bought the Micra as a stop gap. As it's my missus policy she will lose her no claims for this year, understandable, but here is the crunch...
1. Come renewal I have to report that I was involved in a fault accident don't I? But obviously I've not claimed.
2. What does the missus have to put down on the renewal form? Her policy has been involved in an accident, but she wasn't driving, does she have to say anything except another year of 0 no claims?
3. I'm currently looking at lease cars at work and one of the questions on the insurance part is 'Accident Claims in 3 yrs' with boxes for dates and circumstances. I've not claimed for anything is it just badly worded and I need to fill it in anyway to report the accident?
I've no interest in trying to weedle my way out of disclosing information/commit fraud, I am just unsure of who reports what come renewal time/when filling out the lease form. Thanks
