Banned members re-registering

Just curious about this.
When a banned member re-registers under a new name- what are the PH criteria for whether they are allowed to stay or not?
THere seems to be a degree of inconsistency of this

Banned members are not allowed to re-register without our permission and 99% of the time a ban is a ban.

Under very exceptional circumstances we have occasionally let someone re-join but almost without fail they return to their old ways and prove that the original decision was the correct one.
