Short answer is yes, they are worlds apart. The older model feels like a bus to drive and an outdated bus at that, don't get me wrong it is a good, solid car which serves many people well but it feels quite old now.
But if your budget will suit a 15 on model you will be rewarded with a (more) classy, modern feeling car with lower running costs and far superior tech.

The original XC90 is a 14 (?) year old design and it shows.
I've had 2 and while being great family vehicles they are pretty dated now.
If you can stretch to the new then then its worth it, if you can't I would personally either buy a cheaper one (as long as its the 200 bhp engine) or go for an X5.
