Rear Turbo Bolting OFF!!!

Hi guys.. May I ask a question please.
I am noticing that after a track day, my rear turbo bolts off nearly completely from the manifold. It already happened twice and it is always the same one that keeps attached well. The others (1 falls off and the other two are well loosened).
Is this a sign of problems with my turbo?? (I recall when buying it, that they told me they fixed a leaking turbo). Or is it just not attached properly.
