Window winder problem

Can anyone explain why the window winder mechanism on the drivers side has torn it self through the fibreglass? Basically one of the mounting bolts - not the two on the winder or the two that hold the grooved channel, the one on it's own in the middle has torn it's self throught the fibreglass. I'm trying figure out why this has happened. The mechanism did get bit stiff before hand - but not excessivly - I can't see that it is bent or anything - any comments?
