What about Mansell in Canada (1991?).

Yeah, mysterious fault as the team put the air starter in the back of it when it returned to the pits and it fired straight up, allegedly. wink

... and I thought it was Brambilla in his Beta March on Austria's flooded O'ring, but I think he actually got across the finishing line first before rattling down the armco.
So he did win. (Damaging race cars was always a bit of an after thought for the Monza Gorilla).
