Time to slag off some daft planning - I'll start the ball rolling with two spectacularly stupid pieces of design (or alternatively, two delibrate attempts at disrupting traffic)

1) M40 jc10 (A43). Some brainless numbskull decided that the best course of action was to force North and Southbound traffic on the A43 to cross paths on a roundabout. The resulting queues northbound off the M40 are legendary. Seeing as there was already a sliproad from the Services roundabout on to the M40 southbound, the only reason the new one was built (and the slip road closed from the roundabout) could be to cause traffic problems!

2) M4 jc4 (Heathrow) and Cherry Lane Roundabout system. Someone was on acid when this was designed. Due to the sheer volume of traffic using M4 jc4 to/from Heathrow, the traffic backs up to this multi-lane spiders web, causing gridlock.
