brake pad problem

What do you think has caused this. These are DS2500 Ferroda pads fitted about 2000 miles ago to the rear brakes on my 4.5 Cerb. The discs are perfect and run true with no cracks or warping. These pads were fitted by a very well known TVR specialist and they did not use anti rattle shims but put an asbestos thin pad between the pad and piston which obviously just got squashed and fell out. The pads then rattled and it was not untill I went to fiit the anti rattle pads myself that I found the problem.

Probably from rattling around. Just silicone them to the pistons in the caliper. Job done, no rattle, no damage.
