I realise not everyone removes the manifolds to get to the gearbox and clutch. But if the manifolds had these added to the downpipe that would save hours or at least make things much easier. Is there a downside?

Definitely would - and would also allow for adjusting the clearance around the rear of the gearbox too.
ETA - actually, looking at the way the two pieces mate, it looks like there would be no room for adjustment?

I think the only adjustment would be rotational, which might have some limited use, depending on where on the downpipe they were fitted.
