crunchy gear change at higher revs

Can anyone help me?
I have had my cerb for just under a year now(2003 4.2) and by and large it has been trouble free. It is due for a service but recently i have noticed that when changing gear into 4th & 5th i am getting a gentle crunch even when my foot is flat to the floor.
It seams to happen when the engine warm.
1) is it normal to have to press the clutch right down to the floor?
2) is this sympton a sign of something i can investigate myself or is it the syncro mesh hub on it's way out?

Believe that the syncro on 5th is a bit of a weak link.

i've always had this from 2nd to 3rd.. only when the trotter is firmly down. i find a slight pause between helps.

I had this for a fair while, unfortunately it probably means your synchro is on its way out. Mine laste for 20,000 miles doing that though until the gearbox finally went bang on the motorway.
