4.2 AJP timing chain tensioner

Anyone come across a problem with the timing chain tensioner pulling its thread out of the timing cover.. the car came in with an awful rattle/knock esp on rev down, upon inspection i found the tensioner plug and pin hanging on by 2 threads having somehow lost its thread in the cover. I'm wondering if this was damaged previously by overtightening. There is no thread left in the cover now but enough material to tap out an 18 mm thread for a re engineered tensioner.
Any thoughts appreciated.

never seen that before, you'd have to be pretty hamfisted to rip those threads out though

The thread tapped out nicely to 18X2.5 installed the re machined tensioner bolt and she's as sweet as... (a cerbie can be)
