Anybody know about Berlingo 1.6tdi turbo replacement?

Hi, i'll keep it short but basically my mates turbo failed so he bought a new one from a localish supplier. They gave him a long list of parts to change like oil pipes etc. My mate took it to a local mechanic who said a lot of it was unnecesary and said cleaning out the pipes etc would be ok. New turbo on and failed in less than 10 mins. Turbo supplier said this was due to oil starvation, some nut had blown off and the turbo destroyed. Mate buys a new turbo and takes berlingo to a different garage who come recommended by someone else who had a turbo change.
This garage then installs the new turbo taking care to replace every part as per the instructions and gives the van back with the turbo running 100%. Everythings fine for a month until the turbo fails again. This time supplier inspects it and says the fan/propeller thing is broken due to a foreign object gettting into the turbo through the air inlet. Garage says very unlikely as the air inlet pipe has no marks or damage, all the filters etc are in place and had there been something in there from the install then it would have already failed.
Either way nobodys willing to pay for a new one! Anybody got any advice on where to go from here?

The tsb on these has to be followed but as a rule ime it's the oil pick up becoming sludged that causes failure seems a bit odd the second one failed with impact damage though is there any sign of what it could be?? Check the pipes after the turbo & the intercooler as you don't want whatever it is making it into the engine!!
