Sounds like time for a flush and maybe check the water pump, and add a new thermostat to me.

Fans have a habit of partially seizing so they don't spin fast enough either this or maybe the pressure cap has had it

Flushed radiator today which was quite clean.Changed thermostat, filled with fresh antifreeze and bled it.Idled for 20 minutes with no dramas! Fan comes on ok.Wonder if its just been an air lock?
