Slipping the clutch, big loads, driving like you stole it. Clutches fail for a whole host of reasons. Nothing looks to have disintegrated but it is tough to tell from the shoddy photos thus far.
Clearly you have a theory though, why not start with that?

Dual mass failure ruined caused the damaged. As for what caused the dial mass to die, usually the driver (if it’s decent parts)
I used to work on fleet and I’ve seen transits with dual mass flywheels absolutely knackered after only a few k from new. These were extreme though but usually happened to the road worker gangs who’s smash the absolute arse off the vans and tow mini diggers about. Same guys would get through brake pads in weeks!
Just to give you an idea of normality. My own (LWB) transit has had one clutch and flywheel from new after a gearbox fault the dealer warranted. There wasn’t even anything up with the parts but just replaced them while the box was off. It’s now on 178,000 miles and has done 132,000 on the last clutch and flywheel and all fine still. Thing is, it’s my van and I don’t beat the thing.

I'd say the holes are double drilled to accommodate the flaring of the rivet.
Last one I saw where the friction material was off was years ago & involved a Triumph Spitfire.
We'd supplied a complete clutch to a garage customer.
Came back within a fortnight with the driven plate in bits.
Sent out a replacement & we sent the first one to the manufacturer.
The replacement came back in much the same condition as the first but this time the garage, having made a few enquiries knew what the cause was.
The owner of the car was a young female who delighted in dropping the clutch & squealing the tyres. Fine with the original worn clutch but not with the new ones that didn't slip slightly.
She was advised by them that if the latest clutch came back in a similar state she would be charged full price for remove, replace, reassemble.
No further repeats!

pointless showing a picture of holes, and no reference to the plate itself to see if there are corresponding rivet holes.
If you are saying this is a new van...like brand new ? and how many miles ?
It may be a manufacturing defect, the cover doesnt look particularly burned, but all too often clutch failures are down to the driver.
But on such a new vehicle....surely that's warranty ?
