Ford Engined LDV rocker problem

We are running a 52 reg LDV Convoy Minibus at work which is fitted with the Ford Transit DOHC turbo diesel engine. Last week the inlet rocker shaft came off (bolts undid themselves) so we refitted the bolts and started the engine.
When started it was definatley noisier (seemed to 'knock' alot more than usual like tining wasnt quite on) but ran Ok.
This week it returned on the back of a recovery truck and all of the bolts holding the inlet rocker shaft in are sheared off and the rocker shaft was loose in the head.
I cannot understand why it has done this? The only theory I have is that the timing chain has stretched (causing the increased knocking noise) and the pistons were contacting the valves causing the rocker shaft to break free. I didnt think the tollerances would be that precise in a comercial engine but I have drawn a blank to any other causes.
Am I right or is there another avenue to explore?
