54 plate corolla 2.0 d4d surging and struggling to start?

Hi all
My brothers 54 Corolla d4d 2.0 recently has been struggling to start,needing to be turned over a fair while before firing at which point it puffs a load of smoke.Also when driving it lately it has a habit of struggling to pick up speed (as if your in the wrong gear) no matter how much throttle you give it leaving a trail of light coloured smoke then when it gets to around 3000 revs it surges and takes off and drives normally (all be it with puff of grey smoke.It has no engine lights or anything on and iv had two code readers on it which hasnt thrown up any codes but when i did rev it up while idling a few times we had a sudden fog of black smoke blanketing the street.
Could anyone shed any light on this please?

I would start with checking the EGR Valve.
The 2.0 D4-D engine is renowned for problems with the EGR. I do speak from experience and have had two engines replaced on this basis.
I am not tecnically minded but I have a feeling problems with the EGR also had a knock on effect of causing head gasket failure (thus the engine replacement).
Speak with Toyota as they are aware of EGR issues and have been known to replace engines free-of-charge, even out of warranty period as a gesture of goodwill (depending on vehicles service history, mileage etc).
Do a google search on 2.0D4-D EGR Issues/Symptoms and that should throw a little more light on things.

As mentioned before I am not mechanically minded (wish I was)! Try a google on Toyota 2.0 D4D EGR Removal and there are quite a few ideas to get things moving (Avensis forum has pictures too).
I have checked Autodata and the symptoms you have mentioned would certainly point to an EGR fault, if not an Air Mass Meter or even something as simple as an Air Filter could cause this issue.
See how you go but sadly I always have to get a technician on my cars as I usually end up loosing too much patience/skin/blood attempting things myself!
