520d filling problem

A couple of times now I've been filling at garage, and the pump hasn't stopped... fuel just keeps coming...all down the side of the car and all over the forecourt... via my suit trousers and new shoes.
Okay admittedly I should have noticed, but I wasn't really watching.. well not the fuel filling anyway.
Why? What can cause this?

It's not normally a problem I suffer from as my 645 uses the stuff faster than I can fill it!
More seriously some diesel pumps have a clip that you can hold the pump handle trigger down with and that will override any cut off you might expect. They are not difficult to put on if you're not careful, but they are less common than they used to be.
