http://www.pistonheads.com/gassing/topic.asp?h=0&a... should answer most of your questions, or the DSA website for the "what do I actually do on the test" bits.
Short story:
CBT, see if you can actually ride a bike, and play in traffic.
Theory & Hazard Avoidance: point and click jobbie on the Highway code etc, there are numerous threads around on this one.
3-5 days instruction. Riding a bigger bike, practise, practise, practise, a few runs of likely test routes to familiarise yourself (and lots of U-turns)
Test: Shake hands, do test, thank examiner, grin, bung instructor a tenner, and lock bike up for the day and go pub.
Then the learning begins...

As I understand it, if you have a car licence then you still need to take a hazard test for your bike licence. But if you have a full bike licence then you don't need to take the hazard test for a car licence. That could mean that even the DVLA recognise bikers are better at spotting hazards
It really means there are extra, bike specific questions for a bike licence though. Of course, this could have changed since my Brother-in-Law did his DA test about 4 years ago.

Got a car licence so do i need to do another hazard test?
Yes, you do. It's silly as the actual hazard test is still a camera strapped to a car... Think my theory test had 1 bike specific question it...
Perhaps a better name would be the "Theory and Hazard perception tax"
