just watch an episode of all creatures great and small... You have to force your arm in past the elbow with the thought that if it goes in, it will come out. Its fairly easy then

And wear long sleeves. You'll end up with scratches and posssibly 'claret' too otherwise. I did, the first time.

Draping a cloth on the edge of the speaker hole helps too.

I found I needed to insert my arm, 10mm ratchet ring spanner in hand, at a 30-45 degree upwards angle, right as far as it would go, then rotate the arm and hand down to where the nuts are. They are in a straight line, but the rear one is lower than the front, they are not level with the floor all the way back.
Hope this helps.

Also, on my previous car (a 2000 W reg), there were 4 nuts on each door card. On my current '96 model, there are only 3.

A bit of a bonus really.

When refitting replace the nuts with wing nuts.

No messing trying to juggle a ratchet in there next time.
