Removing door trim

Does anyone know how to get the interior door trim off. The window rattles and waves about if I have it part open. I think something hasd come loose but cannot get the trim off. I have removed the screw behind the ash tray and the nut and large washer near where the speaker is fitted but I cannot find any other fastenings but the trim won't come off. What am I missing???

This photo should help you locate the fastenings. Yellow arrows are self-tapper locations, green arrows are studs that have 10mm nuts and large plain washers fitted to them inside the door. Red arrows show holes in card that studs go through.

take the speaker out and use a ratchet spanner - you need to be double-jointed tho...
