To remove the door panels do what it says in the bible but take note that the arm rest is held in with 3 bolts, not one as stated. (Well, it is on mine anyway).
You have to first remove the reflector (if its not already broken off!).
Then remove the speaker and put your hand and forearm through the whole to feel around for the nuts that hold the armrest studs on.
Undo these, remove the armrest and then the rest is fairly easy.
The adjust bolt is packed out with washers and is at the end of the door that opens (not the hinge end), at the top on the inside.
You may find that this isnt the problem though and that your glass has come out of the rubber runners and is being pinched tight. Check and lubricate the rubber runners too.

If you start with the window fully down, look half way along the length of the window (I think on the inside of the glass but I can't be sure, behind the brisles) you should see a hexagonal allen bolt thingy, this is essentially the top stop for the windows - turning this anti-clockwise should lift this and thus make window rise further.
