Small friendly PR company recommendations please

I refuse to pay for shiny veneer and little actual effort, can anyone recommend a small ballsy PR (preferably a 1/3 man team).

Its not a cost issue its a bks v delivery issue (ie less bks more delivery)

In theory "you can do it yourself" to an extent but a good pro gets results.
