Excel help for a charity event

Ok, a good challenge has hit us, but I am in Greece on a project. We are doing a charity drive to Turin in MINI’s and I have need given a free exhibition stand at a trade event to put our mini on and raise money.

We are running a little comp so I need a very simple form created that allows visitors to our charity exhibition stand next week that allows visitors to enter name, mobile, email and a 4 digit number, that is then dumped into a separate page.

Ideally needs to work on a simple iPhone or iPad.

Exhibition opens Wednesday morning!
