Go to Zamzar.com download utility, upload pdf, get back jpg or doc whatever (you select) then deal with in cs3. Simples

Open InDesign, then create a new document IIRC 1 1/2 times the size of your document, place the pdf in, then press 'E', hold shift and rotate 45 degrees, the exterior of the document will be white though...
