Time to get two new tyres for my Alfa GT JTDM, 17" 215/45. Do I just go for another pair of the P Zero Nero that I've been happy with or should I be considering something else?

Also noticed a lot of tyres in this size are "XL" spec and I'm sure that wasn't the case last time around. Do I need XL spec? What does XL mean anyway?

XL means Extra Load and is a reference to the weight of the car, not that you carry big loads in it. It's often seen on low profile tyres on big cars. The extra two numbers at the end of your tyre description are the weight. You haven't shown that on your post.
But your tyres should say something like 225/45/17/91W. The 91 is weight loading and it's important you at least meet the manufacturers recommendation. It should be shown in the handbook.

Lots of people like P Neros. I had them on my 156 and didn't really like them. I thought they felt very wooden and fitment of Goodyear F1s (GSD3 as they were back then) confirmed this. They made a huge difference to the feel of the car. If you're happy with them, stick with them, but if you fancy a change I don't think you'll have a detrimental effect on the car.

The tyres fitted are 215/45ZR17 91Y, handbook says 87W or 87Y

That's fine then. 87W is your minimum spec, anything over that and you are safe.
