Can anyone recommend a quality hosting company?

I have waited two weeks for my current provider to get a server online and they still have not managed!

The requirement would be

A dedicated and fully managed *nix server
MySQL,
PHP4
GDImage.
PHPMyAdmin being pre-installed is also very handy.
Must have very good 24x7 (telephone preferred) support.
Good value for money.

If you no of any company that could fit the bill please let me know asap as I wish to make a decision on this early next week.

If I may put in my twopenn'orth, I took Jamie up on using Serverstream and am very glad I did so. The service is excellent, the support is quick and friendly, and it's altogether value for money. Highly recommended.

there are cheaper about and I'm sure several other PH'ers are hosts as well...
