Im looking at an e55 amg today and its a 2004 with 89k miles, between first and second on take off. Theres a slight hesitation, and a slight small "ek" like a belt slipping. Is the slight bump in gear change just wear and tear or is it something else?

The small ek is a a squeak at about 1500 rpm, if so is is most likely the kompressor clutch engaging. As for the hesitation my SL does not have it at all (same engine and box I believe).

the squeak your hearing is the supercharger. Its pretty much a standard noise & should cause no concerns. Mercedes did actually produce a document a long/long time ago warning dealers not to replace the supercharger as this was a standard charcteristic of supercharged vehicles. Don't know why but all Mercs fitted with superchargers seem to differ in the degree they squeak - some are louder than others & mileage/age/the way its driven does not seem to have any bearing on how each car sounds.
