SWMBO has just started doing this for a few friends using old slate tiles from our shed roof. She has some special waterproof apints and just prints the requisite wording from the computer and copies that. I would say your best bet for an original is to find a local budding artist and some scrap slate and ask for what you want. You'll get something personal to you then and not just a mass produced item.

Cant you just name it privately and use it when the house is being very good and you want to show it how much you loves it?Houses in steets/lanes where they are all named (and have been for 100+ years) is fine....streets that go 1,2,3,4, Donworkin, 6,7,8,9 Love Shack - are nuts!

However, if you want the vanity of post addressed to the house name then you can just register this with the utilities, etc and they will change your address on the system, but it will keep the number as well.
I can understand why you might want to, but just be aware that you can't lose the number entirely, but you can complement it.
Names are a liability, firstly unless they are registered in PAF (postal address file) it won't be recognised by most computer systems so you will still have to use the number. If you have a nice postman you'll still get deliveries, but only because he/she knows where it is, it won't be on any satnav system and unless you live on a small road people won't be able to find you by looking at the numbers of other houses.
Our neighbour decided to do this and sometime later had an accident, the ambulance was driving all around trying to find them and if they had just used their number they would have been fine.
An address is a means of finding you, nothing else, why make it more difficult for people than it needs to be?
