When looking inside the abdomen with a camera (laparoscopy) they use the belly button as a way in. It's already a scar (no new scar) it's about the right size for the laparoscope (10mm) and behind it there is usually nothing important tethered. There can be a remnant of fetal growth leading down from the umbilicus, the Median Umbilical Ligament. This can be cut without consequance.
Once you've opened an abdomen (surgically) you develop adhesions, which stick anything to anything; so in this case, lots can be stuck to the back of the umbilicus
