private plate / v750 - HELP!

Just bought myself a private registration plate & received the Certificate of Entitlement V750 through the post today.
I have my V5, MOT & photocopy of the tax disc, but do I fill out anything on the Certificate of Entitlement V750??
I have read over it and it doesn't make much sense?
Id take it to the DVLA office (closest one is Glasgow) but I work Monday to Friday so dont have a chance to do so!
Section 1 has a signature and a date, guessing from the website which it was bought, but do I fill anything out in this section?
Any help would be grand, cheers!
