Our old Skoda TDI has 150k miles on the clock now and I notice recently, as in last four/five months that more revs are needed in fifth gear than we used to use. Anything less than 1500 revs results in a jerky forward motion, the sort of thing feels like misfiring. It is slowly getting worse and I am a little puzzled. Any suggestions please.

Could be a Maf sensor, does the car still pull ok higher in the rev range aside from the jerkyness/vibrations you are seeing in 5th? When my maff went I strugged to exceed much more than 3000 rpm in anything other than 1st & 2nd.
More likely(assuming performance up the rev range is unchanged) your Dual mass fly is on the way out and is no longer able to do its job properly at lower revs. Is the DMF original at 150k?

Don't think the DMF would feel like that.
Look for small splits in the intercooler pipes

Have you made any progress with this? I have an A4 TDi with a similar albeit much less noticeable issue. It is not just fifth gear I can feel it, any gear at very low rpm feels like a misfire as you describe. It seems to be more noticeable if I have taken a long drive with no issues, stopped for 10mins (for example at a garage) and then restarted. If it had been a petrol, I would definitely be thinking ignition but am wondering about MAF or fuelling now.. (or as a previous poster has already said, maybe a turbo pipe leak)..
