What does it take to get a GoPro suction mount to fall off?

Other than not putting it on properly... Not a lot is going to make it fall off, I believe they're designed a tested to well over 100mph.

Having said that, I use a tether made from para-cord and a small carabiner

the go pro youtube channel has got a video made with it stuck on one of them aerobatic planes.

as long as the surfaces are clean it'll stick bloody well.!

that said i tether mine too... just in case

Taken one up to an indicated 190 and it didn't budge, so I guess it'd take user error, a poor surface, or bad luck

Mounting it on a surface which isn't totally flat. I did it on my car door and it fell. Not that it mattered until another car ran over it. Thankfully the camera itself was fine and only the suction mount and casing needed replacing.
