Spreatsheet to track career achievements

To prepare for performance reviews, I want to (need to) track my career achievements. Does anyone know where I can get a template (excel spreadsheet) that is designed for this?

One of your career achievements could be to create a spreadsheet for tracking career achievements.

Can't you just make it up?

Date | Employer | Position | Project/Achievement | Description | Budget | Deadline | Result (ie like a government department or a private business )?

In addition to capturing the headlines and impact on your CV; if you want to track CPD and employ reflective practice, then most professional bodies provide on-line logs where you list what, where, when, why, so what and most importantly, now what? Check the bodies representing your industry and see what they can offer if you're not already a member.
