Sounds like timing chain

but when I took the cover off my 1293 MED engine the duplex and tensioner seemed fine , but the oil spreader washer was very loose on the crank , is this normal .

It doesn't matter as it's clamped tight by the crankshaft pulley when you do up the pulley bolt.

so why have I got a load rattling noise that sounds like timing chain , anything else I could check ?
