Bespoke trip to the US, where to book?

Apologies if this isn't the answer you wanted, but these days it's not all that difficult to make all the arrangements yourself.
Flights are easy, as are local hotels (expedia is a good place to start). The train can't be difficult either, although I've never booked one in the US.
We do it this way so we get exactly what we want. You don't really save much over a travel agent, but the flexibility is better.

I'd recommend D-I-Y. Missus and I have done this often.
Book everything from the UK before you go. You can use Expedia but I usually decide where I want to be then use Trip Advisor to determine the actual hotel there and then book direct, or maybe via Expedia. Maybe on a road trip you could book the hotels for the first three-four days and then book ahead when in the USA. Easy if you have a laptop of iPad with you.
Re trains, try Amtrak's website: I've used this for a number of rail trips, although mostly in the East USA.

DIY. I’ve travelled to the US many times and always went this way. I used to get quote from an agent then compare that to what I could do it myself for. Hotel policy in the US is usually cancel up to 24hrs before so if plans change or a better deals come up you’ve got that flexibility.
