When I saw it I was convinced it was the clutch release bearing, but if you have changed that and the clutch and still have the same noise, it could be either the first motion shaft in the gearbox or as a very outside chance, the crank case bearing perhaps?!
if youve hed the gearbox disconnected, did you run the engine?( i assume not because the starter will be bolted onto the gearbox....I think?? Ive net been under the V8 for a starter change yet)
I cant remember if the noise was progressive through thhe gears or constant, time for a gearbox change!?
Good luck

Seems unlikely the rattle would then stop when the clutch pedal was depressed at idle...

»» which has a rattle in gear until the clutch is pressed
Right - so you're driving along in, say, 3rd, and there is a rattle.
Then you press the clutch fully and the rattle disappears.
This removes drive from the engine to the box (assuming clutch working OK).
I'm thinking input shaft - there is a bearing on the input shaft and another between input and output shaft (dipped in oil). Could also be (as mentioned above) missing spigot bearing. A mate of mine had one missing and the 'nose' on the casing where the input shaft pokes out, eventually snapped through.
If you can put up with the noise, may as well continue driving until it fails properly. Then it will be obvious.

Stu
thanks for the posts so far
shaun has identified the rattle but firstly its not the gear lever
whilst the body was off we separated the engine/gearbox to rectify the thrust bearing noise but apart we found the springs loose on the clutch plate, this was replaced and a visual inspection deemed the thrust bearing ok.
the car was put back together but the noise seemed the same and through the gears, so the engine came out to replace the thrust bearing with a new but plastic one, car put back together.
this time the noise is there but a bit of vibration too when shifting through the gears (not on the road but jacked up)
we are already to remove the engine again, just so we can get more in petrol to burn it LOL
just looking for thoughts

Let me put this another way: on the 400SX I put up with a number of funny noises, including a whine from the box in 4th and 5th (over and above the whine from the passenger seat and the supercharger), vibrations from the rear at 90 (km your honour), and a clonk from the diff.
However I've racked up 40k miles on it without any of these bits breaking. So I would drive it and fix it when or if it breaks properly. Get an insurance policy that includes free recovery, just in case.
