You think that's bad, I was riding with a mate a couple of weeks ago - the week after I'd told him to get his rear brake sorted as it was off centre and catching - when his rear brake caught my eye for some reason.
I told him to stop and on investigation noticed that what had caught my eye was a long curly piece of swarf that had come off the back wheel as the ally brake block backing had acted as cutting tool.

k knows how much that could be.I was out for a quick spin last summer when a friend was drafting me, came onto a roundabout at about 15-ish and in less than a second there was the sound of a gun going off, followed by a screech of tyre on tarmac, followed by 'ouch, fu**, shi7', and the sound of a middle aged bloke skidding along tarmac on his ass!
Turned out he also had a spoke pop on his rear wheel but he had high tension aero spokes, so when it went ping it sounded like a rifle shot, and due to a low spoke count the wheel immediately became S shaped, it buckled so badly it would not rotate a full turn.
Personally, I had a big failure a couple of weeks ago which resulted in wrecking my rear wheel, rear mech, hanger, and cable - and worse of all my Sunday ride. Although I cannot categorically confirm the cause, it looks like the hanger screw was slightly loose, on an upshift it allowed the rear mech to buckle throwing the bottom of the mech into the wheel - the rest is (expensive) history!
As a word of advice, when you check, clean, service, lube your bike - just check the hanger retaining screw, it could save you quite a bit of cash!

Yup, that's the one - looks like mine had worked loose enough to still hold the hanger onto the frame but allowed movement - and as the rear mech runs close to the spokes evidently it does not take much movement to introduce the mech to the wheel!All fixed now and the screw has been encouraged to stay in place with a blob of locktight!
