Not complaining, just some observations...
I've noticed some issues too… it's coincided with SP2 as well for me, so not entirely sure if this is a server related issue, although SP2 clearly does NOT like the pop-ups.
I've allowed pop-ups on the site, but if you "open in a new window" you often end up with a blank page (also have a nbasty case of "double-click" which is a known threat from many pop-up blockers (for example spybot picks it up). However if you run from one window, then the issue does not occur.

None of the code has changed, the SP2 issue is precisely that - an issue with SP2. I'll see if I can make some changes at some point to make it coexist more happily.

You're still using "double-click" though, which is regarded as a threat by a number of blockers, and increasingly with anti-virus software (especially with heuristics enabled).
Doubleclick is the choice of advertisers for deliverying their inventory. To take the advertising I have to accept their delivery methods.
I don't have a lot of room for manouvre in this respect.
As a user you have the choice not to accept any Doubleclick cookies using your brower settings.

I understand your position... was only making observations...
The world of the web is changing and many previously acceptable ways of doing things are being ousted as they might also be used by hackers. We had our main client demand that all web applications use neither pop-ups or cookies, then had lots of fun redesigning the system to deal with their demands.
Bring back vt100 terminals, couldn't go wrong with that - in fact does PH support Gopher :-)

Ted, are you load balancing your IIS servers by any chance?
Depends on the balancing algorithm in place as to why this works, as I recall Microsoft NLB is just round robin approach so will spot a number of pending requests for the server A and send subsequent requests to server B...
