In my experience you have a much greater chance of injury on rep 7, 8, 9 etc of a lighter set.... that's when your concentration slips. I don't do 1RM attempts that often though

Yes, i can only lift double-overhand up to about 170, and even then i find that my focus shifts to my fingers instead of my back, which ruins the feel of the lift. However if/when i get the 200 in the bag, i am going back down a bit and working on my grip.

Have you ever thought about using straps instead, it always confused me when people say they'd rather lift a lighter weight because of their forearms not being strong enough on an exercise ment for your back, glutes, and quads.However if/when i get the 200 in the bag, i am going back down a bit and working on my grip.

Problem being that grip will never improve if you use straps all the time. Grip can come on very quickly when trained, and will soon stop being the limiting factor in a short period of time IME.Well i managed it, just about! I should probably not have done squats on Monday though.... my second rep i failed to lock out, despite degrees of yelling inappropriate for a public gym 
But one in the bag was enough

Has anyone any good programmes for getting deadlift up? I know myself and PT's have said ive got potential to be doing 250kg now but something isn't firing properly. Ie glutes. Ive done 5x5 and now trying 10rep warm up, 5rep as like a second warm up then 4 sets of 1 rep to my max (and heavier if im upto the job) but only just started this so haven't gone above 180kg yet

Try Coan-Phillipi deadlift rotine. Equally I've had great results from simply doing one heavy 4-8rep set then strip 20kg off and do 10+ reps as a backdown. When you get 6-8 on first set add 5kg to both sets. Doing a lot of supplementary back and hamstring work is v important.
