Am I being nudged out?

Hi guys Iv lurked on here for a few years before deciding this was the best place to ask for advice.
I have worked for my current employer for 3 years ( as of last month) Im a care assistant my job requires me to do 24 hour shifts alone in the homes of the people i care for. Originally I had a few teething issues ( House not being cleaned to the standards of the people who were taking over from me) and these were eventually resolved and after an 8 month probationary period i was confirmed in post. Things were fine up until about september we had recently had some big changes new management (all female after previous male manager and female service leader) after a few weeks one of the other men that work within the team changed to another service and continues to do one shift a week on occasion. After this i started to do more shifts. After a few weeks i got called into the main office for a "chat" where i was told there were several issues (medication counting cleaning and finance being the main ones) I agreed these must be sorted so got my act together. After a few weeks this was repeated ( in total i had 4 meetings about this ) I then got a verbal warning from the company director due to these issues. That was in january since then i know i have certainly improved ( I do not deny the original issues bar the finance one) but i am still being called in and told there are issues. Tonight i have been told there is an issue yet again which is what has prompted me to write this post.
My main concerns are since i have had a disciplinary about these issues and i have improved could i be being nudged out? The reason i think this is because the team has became 2 men and 4 women the women are all friendly with each other and hang about during and after work ( Female staff members will call each other and request to meet for lunch etc) I enjoy my job and strive to do the best i can on every shift but recently it seems to not be good enough and its affecting my work and private life as i sacrifice quite a lot of my own time (especially weekends I havent had one off since January yet every other staff member has) to cover shifts where people have became ill or not turned up.
So should i be looking for another job/Service or should I just man up and keep trying?

If they were trying to nudge you out on false pretences I don't think they would have had so many meetings with you. Quite frankly it's a PITA to recruit care workers (which I guess they would need to do if you left?). far better to work with the ones already in post.
I would suggest you meet with your Line manager, go through/document the areas for improvment, set tangible/measurable improvement targets and a minimum 3 month review period.
You may be able to demonstrate how you have improved. Equally they might be able to demonstrate where yoou are not up to standard.

Explain 24 hour shift? you can't be on the clock all the time ?
On the one hand you say they might be looking to move you out, yet on the other you get extra shifts and have to cover others.
Ask for a weekend off, or ask when you might get a weekend off. Also ask your manager how long your disciplinary will remain "active". I think it is 12 months, but that might just be policy at my place.
