Any idea as to how this chap has filmed this video?

I've got a app on my phone that does time lapse and another that does tilt shift but no idea how to do moving tilt shift videos? Or even if it's doable.

I can't watch the video (I'm on the top of the North York Moors) but using a tilt and shift lens for video is easily done, rather than using software to emulate the effect of a lens.

havent checked the how to but it seems he was using an actual tilt-shift lens (or at least tilt lens) on the video camera/dslr. It wouldnt require anything in the way of processing.
