You can quite easily get 386mm discs under a 18 rim, not sure what car would need to run bigger than that.
That's a small 386 or a big 18 in that case, I can't run 18's on my 380's as the brakes get hot and then rub against the wheels, I've tried numerous wheels. I don't WANT to be on 19's but a set of 19 tyres is cheaper than going through about 4 sets of wheels trying to find a set that fits. What car they going on that runs discs that big

It can depend on the clearence on the wheels, also HI spec calipers seem a little more 'bulky' than AP's, also depends if they are lug mount or radial.
