I've got a worn NSF lower ball joint and have always replaced suspension parts in pairs so that each side feels similar.

Wise or just wasting cash?

Wise (imho)

guess one side could fail because gaiter split and let water in and other be fine but then again that gaiter maybe on the point of splitting ?

Always (nearly always) had things done in pairs.
That said, it might easily be argued that a component that can be fully inspected/tested and is known to be in good condition could be left in place.
Best example I can think of that, would be a wishbone.

I would suppose it is just considered good practice, especially when the component is considered 'throw away', as I would suggest a ball joint is.

In general then, I would agree with phillpot and phazed.
