I am a very keen cyclist, commuting 30 miles or so everyday, most days of the week. I seem to have developed a nasty knee pain which has almost entirely stopped me from riding lately.

The pain starts on the inside of the knee and then develops through the middle/underneath of the knee cap. It is not really torque dependent, i.e it hurts on rotation of the pedal stroke, not necessarily when im pushing hard.

I can squat in comfort with toes pointed out slightly, but rotating toes inwards puts a large amount of pain at the top of the thigh. This area feels very tight - does this sound like my problem? Is there any way I can relieve this tightness? I think this may be the problem connected to the knee pain.

Any advice greatly appreciated!

I had something similar, turned out to be this;

A good ostepoath will probably diagnose instantly.

Might be worth getting a bike fit. Particularly if you are riding a road bike which are harder to get into a comfortable position in without a proper set up.

I've been riding with the same setup for years, nothing has changed and I am completely comfortable with it having covered thousands of k's!
