Got Private plate docs - Need a V750 to put on a car?

I think I need a V750 form to transfer the plate onto my M3. Where the hell is one of these on the internet?

No link and cant even find one on the DVLA website. Also no local office so I need to send documents to Swansea?

Another 4 week wait!
